Student and Academic Information Forms
To request access to these forms, please have your supervisor fill out the request form at the Registrar's Office. One special note is for SOAHOLD, you need to explicitly request either the IE hold flag or VLP hold flag.

| SGASTDN | The General Student form (SGASTDN) encompasses many details about a student. Everything from residency status to participation in Greek Life is housed within the tabs of this form. | None |
| SHAINST | The Academic Status Form (SHAINST) is another method to view a student’s Academic Status term by term and Academic Status. In addition this form displays if a student received the Dean’s List certification for a given semester. | |
| SFAREGQ | The Registration Query Form (SFAREGQ) is used to view a student's registration. It lists all the classes for which a student is registered, the meeting times, the building and room of each class, and the course campus code. | |
| SFAREGS | Used to add, drop, or withdraw a student from a course or courses. | |
| SFARHST | The Student Registration History Form (SFARHST) can be used to view general information about a student’s registration. SFARHST will only display the CRN, Subject, Course Number, and Registration Status Code, so if an Instructor, Time, or Location, is needed, refer to SFAREGQ. SFARHST will show if a student drops or withdrawals from a course. | |
| SGASADD | The Additional Student Information Form (SGASADD) maintains information about student cohort codes, as well as student attributes. SGASADD is essentially two Detail Blocks, one for Cohorts and one for Attributes. It is important to remember that each block is term specific. You will most likely need current term, but note that the data you see within one term, 201408, may not apply in the next term. | |
| SOAHOLD | In Banner, a flag that is placed on a Student’s account is called a hold. As in IMS, there is a start and end date for each hold. The start date may be the current date or a future date. Unlike in IMS, Banner requires an end date. The end date cannot be left blank. Banner will automatically populate the date, DEC-31-2099, in the end date field. Please leave this date for any hold that you wish to remain open indefinitely. | |
| SHANCRS | Use the Academic Non-Course Form (SHANCRS) to view academic non-course information related to a student, such as High School Curriculum (RHSC) requirements (formerly called College Preparatory Curriculum or CPC). | |
| SHASUBJ | The Subject Sequence History Form (SHASUBJ) displays summary information for a student for each subject area, with hour and GPA totals by subject. Both institutional and transfer work display. | |
| SHATERM | The Term Sequence Course History Form (SHATERM) displays academic history by term. Academic history information for the student is displayed for the term in the key block and other terms in ascending order. If no term is entered, all academic history records of the student. | |
| SIAASGQ | Use the Faculty Schedule Query Form (SIAASGQ) to view faculty schedules for a term. | |
| SFASLST | The Class Roster Form (SFASLST) displays section information and the students enrolled for the section. | |
| SOATEST | The Test Score Information Form (SOATEST) is used to add, maintain, and view test scores information. This form shows raw test scores. It does not show course credit awarded from testing. | |
| SPAIDEN | Use the Identification form (SPAIDEN) to view contact information. | |
| SFASRPO | Registration permits are codes that can be put on a student’s record that will permit the student to register for a specific class for which they would not normally be eligible. These codes DO NOT register the student for the class. Rather, once on the student’s record, they permit the student to request the class via Athena. | |
| SSASECT | for sectioning officers of courses; For OIE purposes these are most often for SABD and STEX . Plus, the academic courses for Cortona, Oxford and Costa Rica. | |
